New Patient Request

We are only paneled with Blue Cross Blue Shield of Louisiana. We require a $100 deposit upon scheduling initial appointment that is reimbursable depending on copay.

Self pay rate is $250 for initial visit included in that is a required$100 deposit upon scheduling initial appointment. Every follow up is $200 upon appointment arrival.
